Procedures to Request Indication, Correction, and Deletion of Personal Information
If you would like to request indication, correction, or deletion of your own personal information, please fill out the applicable form and send it to the contact address shown below with required documentation to certify your personal identification enclosed. We will first authenticate your identification and then accommodate your request. Please note that you shall bear postage and other expenses incurred in sending your request.

I. Personal Identification Certification
Please send either
i) a copy of your passport,
ii) a copy of your driver's license, or
iii) a copy each of any two of either your; health insurance card, pension handbook, booklet for the physically or mentally handicapped, or resident's card.
*We will not return any submitted copies of your identification but retain such documentation under lock and key for our records.
*For a request for indication of personal information, the information will be provided to you personally by hand or by mail. In case of mail delivery, 500 yen will be chargeable to you.
